Write your appeal letter

Start with the reason in your denial notice. Check every field, add the facts only you know, and use public information only when it clearly applies to your insurance.

Before you send it

  1. Check the deadline printed on your denial notice.
  2. Use the address, fax number, website, or form named in that notice.
  3. Attach the notice and any medical records or doctor’s letter that support your request.
  4. Save a copy of everything you send.
  5. Save proof that the insurer received it, then write down when it owes you an answer.

If the notice does not say where to send an appeal, call the number on your insurance card and ask for the instructions in writing.
